Referral Information & Consent
La Cura Collective accepts referrals from community partners, schools, service providers, and families to connect youth with our programs and supports.
When you submit a referral, we may collect:
• Your name and contact information
• Your relationship to the youth
• The youth’s first name and age range
• A brief description of the reason for the referral
Referral information is used only to review eligibility, coordinate services, and follow up with the youth, their caregivers, and referring partners. By submitting a referral, you confirm that you have the appropriate consent to share this information.

Our hours of operation are:
Monday: 9:00 AM – 5:30 PM
Tuesday: 9:00 AM – 5:30 PM
Wednesday: 9:00 AM – 5:30 PM
Thursday: 9:00 AM – 5:30 PM
Friday: 9:00 AM – 5:30 PM
We are currently closed on Saturdays and Sundays.
